Welcome to the t-shirt order form (shirt orders are coming to a close soon, so order now)

Here are some important points to consider before you place your provisional order:

• The Original Shirt is available in Red Only
• We must order 24 shirts per design
• Once we have the reached the minimum order of 24, payment must be made in advance to the Treasurer (Alan) before we place your order for production
• Your first name and call sign will be above the pocket unless you opt-out
• Shirts can be ordered by club members, fellow Hams, friends and family members and other interested parties
• All shirts are high quality cotton and can be ordered in three different colors (Black, Charcoal and Dark Blue) and in various sizes (S-5XL)
• Filling out the form below indicates your intent but the order is only placed once Alan has your final OK and payment
• If you want more than one shirt or are ordering for someone else, use a second form (in other words - one shirt per submission)
• Pricing is subject to change based on the final design parameters and when we place our order, however we believe these prices to be very close
• Please double check your information, what you put here will end up on your shirt, might be hard to explain why your name is misspelled
